Our dementia specialist <a href=\"https:\/\/www.dementiauk.org\/information-and-support\/how-we-can-support-you\/what-is-an-admiral-nurse\/\">Admiral Nurses<\/a> explain how to arrange it and how it could help you find support as a carer. <\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">We are aware that in some areas, there are waiting lists for an assessment. While there isn\u2019t a direct substitute for having a carer&#8217;s assessment, our Admiral Nurses can help identify your most pressing needs and suggest a plan while you are waiting.<\/span><\/p>\n<h2 class=\"pf0\"><span class=\"cf0\">What is the carer&#8217;s assessment?<a id=\"about\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf0\">A carer&#8217;s assessment is an evaluation of your needs to determine how your caring role affects your life and what support you may need to make it easier. Your local council\/local authority is responsible for carrying it out.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">Even if you feel you are coping well, it\u2019s still worth requesting a carer&#8217;s assessment. It means support can be put in place to help you keep caring for the person with dementia for as long as you want to.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">There is another type of assessment called a needs assessment, which examines what support the person with dementia requires, rather than what you need as a carer.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">These are sometimes done at the same time, but if possible, it\u2019s best to arrange them separately so you can focus on your individual needs at the carer&#8217;s assessment.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Am I a carer?<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">Often, people who look after a family member or friend with dementia don\u2019t consider themselves to be a carer, but you are a carer if you help the person with:<\/span><\/p>\n<ul>\n<li class=\"pf0\"><span class=\"cf0\">washing<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">dressing<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">taking medicine<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\"> getting out and about, including going to medical appointments<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">shopping, cleaning and laundry<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">managing bills and finances<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">providing the person with company<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">watching over them if they cannot be left alone<\/span><\/li>\n<\/ul>\n<h2 class=\"pf0\"><span class=\"cf0\">Support available for carers<a id=\"support\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf0\">There is a range of support available for carers, including financial assistance, support groups and resources to help you connect with others who understand their challenges. The carer\u2019s assessment can help you identify what you might be able to access.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Financial support<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">Carer\u2019s Allowance is a benefit designed to support carers, currently \u00a383.30 per week (this is reviewed annually in April). You must be caring for someone for at least 35 hours a week, and they must be receiving certain benefits such as <a href=\"https:\/\/www.dementiauk.org\/information-and-support\/financial-and-legal-support\/attendance-allowance\/\">Attendance Allowance<\/a> or Personal Independence Payment (PIP). There is an earnings threshold. For more information visit the <a href=\"https:\/\/www.gov.uk\/carers-allowance\" target=\"_blank\" rel=\"noopener\">Government\u2019s Carer\u2019s Allowance page<\/a>.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">In Scotland, the equivalent benefit is called <a href=\"https:\/\/www.mygov.scot\/carer-support-payment\" target=\"_blank\" rel=\"noopener\">Carer Support Payment<\/a>.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Respite<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">If your carer\u2019s assessment identifies that you need a break from caring, your local authority may recommend respite care for the person with dementia. This might involve having a paid carer or volunteer to sit with the person for a few hours, a regular place in a day care centre, or a short stay in a care home. <\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Training<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">A carer\u2019s assessment may suggest that you receive some training to improve your coping skills and teach you how to best support the person with dementia. This might include education on: <\/span><\/p>\n<ul>\n<li class=\"pf0\"><span class=\"cf0\">how to safely handle and move a person with mobility problems<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">dementia awareness<\/span><\/li>\n<li class=\"pf0\"><a href=\"https:\/\/www.dementiauk.org\/information-and-support\/health-advice\/medication-for-dementia\/\"><span class=\"cf0\">medication management<\/span><\/a><\/li>\n<\/ul>\n<p class=\"pf0\"><span class=\"cf0\">Training may be offered as a structured programme or one-to-one guidance.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Access to support groups<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">There are many support groups that you may be able to access as a carer, regardless of whether you have a carer\u2019s assessment. These can be a great source of advice and support from other people who care for someone with dementia or another condition. Support groups may be local and in person, or online.<a href=\"https:\/\/carers.org\/our-networks\/the-carers-trust-network\" target=\"_blank\" rel=\"noopener\"> The Carers Network<\/a> can help you find networks within your area.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Home help<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">The carer\u2019s assessment might recommend that you have some help at home. This could be, for example, a paid carer to support the person with dementia and give you a break, or help with housework or gardening.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Help with travel costs<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">The carer\u2019s assessment might recommend that you receive financial help with transport costs, such as taxi fares, if you don\u2019t drive.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Mental and physical wellbeing support<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">Many carers find their mental and physical health suffers because of their caring responsibilities. The carer\u2019s assessment might recommend strategies to support your wellbeing, such as gym membership or exercise classes.<\/span><\/p>\n<h2 class=\"pf0\"><span class=\"cf0\">Eligibility for a carer&#8217;s assessment as a dementia carer<a id=\"eligibility\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf0\">Anyone over 18 who is providing care for another adult is entitled to a carer\u2019s assessment. It doesn\u2019t matter how many hours a week you spend caring, or how much you earn.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">The person with caring responsibilities must consent to a carer&#8217;s assessment. If you are requesting it on behalf of a family member or friend, ensure you have their permission.<\/span><\/p>\n<h2 class=\"pf0\"><span class=\"cf0\">Requesting a carer&#8217;s assessment in England<a id=\"request\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf0\">In England, you need to request a carer\u2019s assessment from your local authority.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Contacting your local authority<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">If you are caring with someone with dementia, you\u2019ll need to contact Adult Social Services within your local authority to ask for a carer&#8217;s assessment. You can search for your <a href=\"https:\/\/www.nhs.uk\/service-search\/other-health-services\/local-authority-adult-social-care\/\" target=\"_blank\" rel=\"noopener\">local authority on the NHS website<\/a>.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">Referral from a GP or social worker<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">Your GP or another medical professional, such as a nurse, can refer you for a carer&#8217;s assessment if they think it would be helpful, and you agree. <\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf0\">The Care Act 2014<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf0\">The Care Act 2014 sets out how adult social care should be provided and outlines the following:<\/span><\/p>\n<ul>\n<li class=\"pf0\"><span class=\"cf0\">how local authorities should carry out both carer\u2019s and needs assessments <\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">how local authorities should determine eligibility<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">how local authorities should charge for both community and residential care &#8211; if at all<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">the local authority\u2019s obligations <\/span><\/li>\n<\/ul>\n<p class=\"pf0\"><span class=\"cf0\">It takes into account the impact on carers, such as emotional strain or having multiple caring roles, and also emphasises the importance of a \u2018whole family approach\u2019 which considers the impact of caring responsibilities on children and other family members.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">Carer\u2019s assessments should follow the principles of the Care Act 2014.<\/span><\/p>\n<h2 class=\"pf0\"><span class=\"cf0\">Carer&#8217;s assessments in Scotland, Northern Ireland and Wales<a id=\"nations\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf0\">There are some differences between carer&#8217;s assessments in Scotland, Northern Ireland and Wales. You can find out more about these changes by visiting <a href=\"https:\/\/www.carersuk.org\/help-and-advice\/practical-support\/carer-s-assessment\/\" target=\"_blank\" rel=\"noopener\">Carers UK<\/a>. <\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">In all of the devolved nations, you will need to request a carer\u2019s assessment from your local authority\/trust. You can find its details by clicking the links below: <\/span><\/p>\n<ul>\n<li class=\"pf0\"><a href=\"https:\/\/www.mygov.scot\/contact-local-council\" target=\"_blank\" rel=\"noopener\"><span class=\"cf2\">Scotland<\/span><\/a><\/li>\n<li class=\"pf0\"><a href=\"https:\/\/www.nidirect.gov.uk\/contacts\/health-and-social-care-trusts\" target=\"_blank\" rel=\"noopener\"><span class=\"cf2\">Northern Ireland<\/span><\/a><\/li>\n<li class=\"pf0\"><a href=\"https:\/\/www.gov.wales\/find-your-local-authority\" target=\"_blank\" rel=\"noopener\"><span class=\"cf2\">Wales<\/span><\/a><\/li>\n<\/ul>\n<h2 class=\"pf0\"><span class=\"cf2\">Completing the carer&#8217;s assessment<a id=\"complete\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf2\">A social worker will contact you to arrange a time for the assessment. It usually takes place at home but may be done by phone or video call. It generally takes around an hour. <\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf2\">Preparing for the assessment<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf2\">You will be sent a form to complete in advance of the assessment, or with the help of the social worker at the appointment. It is a good idea to review the form in advance to understand what will be asked, even if you don\u2019t complete it until the day of the assessment. <\/span><\/p>\n<p class=\"pf0\"><span class=\"cf2\">You will need to provide: <\/span><\/p>\n<ul>\n<li class=\"pf0\"><span class=\"cf2\">your contact details, including your email address <\/span><\/li>\n<li class=\"pf0\"><span class=\"cf2\">your NHS number <\/span><\/li>\n<li class=\"pf0\"><span class=\"cf2\">the name and address of your GP <\/span><\/li>\n<li class=\"pf0\"><span class=\"cf2\">the name, address and NHS number (if possible) of the person you care for <\/span><\/li>\n<li class=\"pf0\"><span class=\"cf2\">contact details of anyone who will be with you at the assessment<\/span><\/li>\n<\/ul>\n<p class=\"pf0\"><span class=\"cf2\">Before the assessment, take some time to think through everything you do as a carer and make notes to remind you on the day.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf2\">Questions you\u2019ll be asked<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf2\">The social worker should ask you:<\/span><\/p>\n<ul>\n<li class=\"pf0\"><span class=\"cf0\">what the pattern of a normal day is like<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">whether you provide any care overnight<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">what practical tasks you help the person with, eg washing, dressing, cooking, helping them eat and drink, getting them into and out of bed<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">whether you get any time to yourself, away from the person with dementia<\/span><\/li>\n<\/ul>\n<p class=\"pf0\"><span class=\"cf0\">Give as much detail as you can about how caring is impacting your life. The more specific you can be, the more accurate the assessment will be in identifying your needs and what support you require. <\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">The social worker might also look around the home to see if you would benefit from any adaptations or <a href=\"https:\/\/www.dementiauk.org\/information-and-support\/living-with-dementia\/living-aids-and-assistive-technology\/\">living aids<\/a> to help in your caring role, such as equipment to help the person to use the shower or toilet or get into and out of bed.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">It can be helpful to have someone with you during the assessment, as they can share their insights into what your caring role involves and how it affects you. This could be the person you care for, a friend or relative. An <a href=\"https:\/\/www.nhs.uk\/social-care-and-support\/help-from-social-services-and-charities\/someone-to-speak-up-for-you-advocate\/\" target=\"_blank\" rel=\"noopener\">advocate<\/a> may also be able to help you and speak up on your behalf.<\/span><\/p>\n<h2 class=\"pf0\"><span class=\"cf0\">Urgent needs while you are waiting for your assessment<a id=\"urgent\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf0\">If your need for help becomes more urgent while you are waiting for your assessment, you can:<\/span><\/p>\n<ul>\n<li class=\"pf0\"><span class=\"cf0\">Get back in touch with the local authority and stress that the need has become urgent<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">Read our <a href=\"https:\/\/www.dementiauk.org\/information-and-support\/resources\/our-leaflets\/\">advice leaflets<\/a> for useful tips on supporting someone with dementia and looking after yourself as a carer<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">Contact your GP, or someone else who could refer you for an assessment, and inform them of your current situation or concerns<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\"><a href=\"https:\/\/www.dementiauk.org\/information-and-support\/how-we-can-support-you\/\">Speak to one of our Admiral Nurses<\/a> for advice and support<\/span><\/li>\n<\/ul>\n<h2 class=\"pf0\"><span class=\"cf0\">Care and support plan<a id=\"plan\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf0\">After the assessment, the social worker will prepare a care and support plan (which you might hear described as a carer\u2019s support plan) with suggestions to help you with some of the challenges you might be facing.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">This could include:<\/span><\/p>\n<ul>\n<li class=\"pf0\"><span class=\"cf0\"> respite care for the person with dementia to give you some time to yourself <\/span><span class=\"cf2\">\u2013 this could be short \u2018pop-ins\u2019 from a carer, access to a day centre, or overnight\/short-term stays in a care home<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">training in care skills, eg safe ways to help the person move around<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">home help from a carer<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">adaptations or living aids to help you in your caring role<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">carers\u2019 support groups<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">advice about benefits<\/span><\/li>\n<\/ul>\n<p class=\"pf0\"><span class=\"cf0\">The care and support plan is designed to improve your quality of life. You should be asked for your thoughts, so make sure you share any views about what would or wouldn\u2019t be helpful with the social worker.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">Bear in mind that any changes may have to be introduced slowly and carefully to allow the person with dementia to feel comfortable, especially if you are introducing a new carer.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">Your local authority may be able to offer financial help to implement the support you need. It will arrange a financial assessment (means-test) after the carer\u2019s assessment to see what <\/span><span class=\"cf2\">\u2013 if anything \u2013 you are eligible for.<\/span><\/p>\n<h2 class=\"pf0\"><span class=\"cf2\">If your assessment is denied or is inadequate<a id=\"denied\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf2\">Sometimes, the carer\u2019s assessment concludes that the person doesn\u2019t need any support. If this is the case, and you disagree, there are some things you can do. <\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf2\">Challenging the decision<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf2\">If you disagree with the decision made about your care and support needs following your carer&#8217;s assessment, or with how the assessment was carried out, you can challenge this. <\/span><\/p>\n<p class=\"pf0\"><span class=\"cf2\">Initially, you can complain directly to your local authority. It should have a formal complaints procedure on its website, or you can phone and ask to be directed to the correct team.<\/span><\/p>\n<h3 class=\"pf0\"><span class=\"cf2\">Right to appeal<\/span><\/h3>\n<p class=\"pf0\"><span class=\"cf2\">If you are unhappy about how the local authority handles your complaint, or the outcome, you can escalate it to the <a href=\"https:\/\/www.lgo.org.uk\/\" target=\"_blank\" rel=\"noopener\">Local Government and Social Care Ombudsman.<\/a> An ombudsman is an independent person or agency that investigates complaints about organisations.<\/span><\/p>\n<h2 class=\"pf0\"><span class=\"cf2\">Reassessment and ongoing reviews<a id=\"reviews\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf2\">Local authorities must review a person\u2019s care and support plan regularly to ensure it continues to meet their needs. You should be fully involved in any review or reassessment.<\/span><\/p>\n<h2 class=\"pf0\"><span class=\"cf2\">Frequency of reviews<a id=\"frequency\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf2\">There is no fixed timescale for reviewing care and support plans, but reviews will typically be done after three months, and then annually. You can request a review at any point if you are unhappy with the support you are receiving.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf2\">A reassessment should also be done if: <\/span><\/p>\n<ul>\n<li class=\"pf0\"><span class=\"cf0\">There is a significant change in the person\u2019s circumstances<\/span><\/li>\n<li class=\"pf0\"><span class=\"cf0\">The local authority believes the current plan may no longer be appropriate<\/span><\/li>\n<\/ul>\n<h2 class=\"pf0\"><span class=\"cf0\">Young carer\u2019s assessments<a id=\"young-carers\"><\/a><\/span><\/h2>\n<p class=\"pf0\"><span class=\"cf0\">Young carers under the age of 18 are entitled to a young carer\u2019s assessment if they or their parent\/guardian requests it. As with the adult carer\u2019s assessment, this should be requested from the local authority.<\/span><\/p>\n<p class=\"pf0\"><span class=\"cf0\">The young carer\u2019s assessment will consider whether it is appropriate for the young person to be caring, and their feelings about their role.
